Battery health is a simple ratio: your battery's current full-charge capacity divided by its original design capacity, shown as a percentage. A phone that shipped with a 5,000 mAh battery but now only holds 4,300 mAh at a full charge is at about 86% health. That missing capacity is why an older phone needs charging earlier in the day even though your usage hasn't changed.

Health is about capacity, not the battery percentage you see in the status bar. The status bar shows how full the battery is right now; health shows how big the "tank" has become after wear.
